Juniper Health patients can now schedule their appointments entirely ONLINE!
It’s easy! Click the button below and follow these directions:
- Log in to your FollowMyHealth Patient Portal OR create an account if you don’t already have one*.
- Find “Appointments” in the upper right hand corner of your screen.
- Click “Schedule”
- Choose your appointment type from the list.
- Choose your provider from the list OR if your provider does not offer this appointment type, select either “Would you like to change your appointment type?” or “Would you like to find a provider?” If you opt to find a provider, choose one from the list.
- Select “Next Available” or “View Calendar,” depending on your preference and need.
- Select a date or “Continue”
- Provide a brief explanation with reason for appointment and click “Schedule Your Appointment”
Now you’re on your provider’s schedule! You will receive appointment reminders either by e-mail or text message and a prompt to complete registration paperwork from your device.
*If you have recently created a FollowMyHealth account, there may be a 12-24 hour delay in online scheduling functionality.
**If you are a new patient requesting access and haven’t been contacted by someone within 48 hours, please contact Medical Records.
***Dental is not currently offering online appointment scheduling. Please call your local clinic to make an appointment.
