Schedule an Appointment

Juniper Health patients can now schedule their appointments entirely ONLINE!

 

*If you are a new patient, please contact your local office to establish an account before you are able to schedule online.

 

If you are an existing patient, click the “MAKE AN APPOINTMENT” button below and follow these directions:

  1. Click “Continue to Destination” or wait to be re-directed.
  2. Click “Next”
  3. Answer Question 1 then select “Next”
  4. Select “No”
  5. Answer Question 2, then select “Next”
  6. Answer Question 3, then select “Next”
  7. Select an Appointment Date
  8. Select an Appointment Time
  9. Select “Book”
  10. The next screen will ask you for patient information. Complete these fields. You will be sent a verification code either via text or phone call. You will input the verification code on the next screen, then your appointment will be confirmed.

You will receive appointment reminders either by e-mail or text message and a prompt to complete registration paperwork from your device.

**Dental is not currently offering online appointment scheduling. Please call your local clinic to make an appointment.

Get the Self-Scheduling App on your iPhone

Follow the directions for your browser below to create a shortcut to the Juniper Health Self-Scheduling App on your iPhone:

  • Safari
    1. Go to the Safari app  on your iPhone.
    2. Go to www.juniperhealth.org, then click the “MAKE AN APPOINTMENT” button.
    3. Click “Continue to Destination” or wait to be re-directed.
    4. Tap  in the menu bar.
    5. Scroll down the list of options, then tap “Add to Home Screen”.
    6. If you don’t see Add to Home Screen, you can add it. Scroll down to the bottom of the list, tap Edit Actions, then tap  Add to Home Screen.
  • Chrome
    1. Go to the Chrome app  on your iPhone.
    2. Go to www.juniperhealth.org, then click the “MAKE AN APPOINTMENT” button.
    3. Click “Continue to Destination” or wait to be re-directed.
    4. On the right of the address bar, tap Share .
    5. Find and tap Add to Home Screen.
    6. Confirm or edit the website details and tap Add.
  • Edge
    1. Go to the Microsoft Edge app on your iPhone.
    2. Go to www.juniperhealth.org, then click the “MAKE AN APPOINTMENT” button.
    3. Click “Continue to Destination” or wait to be re-directed.
    4. Tap the three horizontal dots menu at the bottom.
    5. Scroll to find and tap Share.
    6. Select Add to Home Screen from the share menu.
    7. Name the shortcut and tap Add
  • Firefox
    1. Go to the Firefox app on your iPhone.
    2. Go to www.juniperhealth.org, then click the “MAKE AN APPOINTMENT” button.
    3. Click “Continue to Destination” or wait to be re-directed.
    4. Tap the menu button in the address bar.
    5. Select Add to shortcuts.
    6. In the share menu, tap Add to Home Screen.
    7. Edit the name if desired, then tap Add in the top right corner
