Following the completion of our recent Operational Site Visit, it was determined that Juniper Health, Inc. demonstrated full compliance with all Health Center Program requirements, an achievement that reflects a dedication to excellence, operational integrity, and continued service to our patients and community. Juniper Health, Inc. received the 2024 Operational Site Visit Badge, a distinction that honors our steadfast commitment to compliance, quality, and the mission of the Health Center Program.

Criteria to earn Heart Health badge: Meet or exceed three of the four measure targets, which are based on the Million Heart Goals (PDF – 976 KB), including tobacco use: screening and cessation intervention, Ischemic Vascular Disease (IVD): use of aspirin or another antiplatelet, statin therapy for the prevention and treatment of cardiovascular disease, controlling high blood pressure (hypertension control).

Criteria to earn Diabetes Health badge: Meet or exceed two of the three measure targets, including adult BMI screening and follow-up plan, weight assessment and counseling for nutrition and physical activity for children and adolescents, and diabetes hemoglobin A1c (HbA1c) poor control (>9.0%) – inverse measure (uncontrolled diabetes).

Criteria to earn Behavioral Health badge: Meet or exceed two of the three measure targets, including depression remission at twelve months, screening for depression and follow-up plan, and screening, brief intervention, and referral to treatment (SBIRT).

Criteria to earn High-Value Care Badge: Meet or exceed the Quality Score: 18.0 (Top 20% of 2023 and 2022 UDS) AND Meet or fall below the Percentage Cost Growth Benchmark: 2.54% (Top 30% of 2023 and 2022 UDS)

Criteria to earn Preventive Health Badge: Meet or exceed five of the ten measure targets, including dental sealants for children between 6-9 years, weight assessment and counseling for nutrition and physical activity for children and adolescents, childhood lead test screening (ages 9 to 72 months), childhood development screenings and evaluations (limited to patients aged 0 – 5 years old), screening for depression and follow-up plan, tobacco use: screening and cessation intervention, adult Body Mass Index (BMI) screening and follow-up plan, breast cancer screening, cervical cancer screening, and colorectal cancer screening.
