History
The mission of Juniper Health is to improve the health of our service area residents by delivering high-quality, patient-centered healthcare regardless of the patient’s ability to pay.
Funded in December 2004 by the Health Resources and Services Administration (HRSA) as a Section 330(e) New Access Point/New Start Consolidated Health Center Project, Juniper Health is a private, not-for-profit Community Corporation that provides primary medical and dental care and a variety of other health services to the residents of Breathitt, Elliott, Lee, Menifee, Morgan, and Wolfe counties in the Appalachian region of southeastern Kentucky. Licensed as a Primary Care Center by the state of Kentucky, Juniper Health also has the distinction of serving as a Federally Qualified Health Center (FQHC) that is partially supported by HRSA grant funds.
The organization was created to assure the availability of primary and preventive medical care, dental care, behavioral health services, health education and social outreach in an open access model. The vision of Juniper Health is to be the area leader in high-value healthcare and its preferred employer, while improving the region in which we serve.
